Age-by-Age Dental Milestones

0–6 months

Before the First Tooth

🎯 Milestone: First tooth usually appears around 6 months

  • Wipe gums with a clean, damp cloth after feeding
  • Avoid putting baby to sleep with a bottle of milk or juice
  • Start looking for signs of teething: drooling, fussiness, swollen gums
  • Schedule the first dental visit by age 1 (or when the first tooth appears)

6–12 months

First Teeth Emerge

🎯 Milestone: Baby should have 2-4 front teeth by 12 months

  • Start brushing with a baby toothbrush and rice-grain-sized smear of fluoride toothpaste
  • Schedule the first dental visit if you haven't already
  • Offer cold teething rings (not frozen) for teething discomfort
  • Avoid sugary foods and juices — water is best between feeds
  • Lower central incisors appear first, followed by upper central incisors

1–3 years

Toddler — Full Baby Teeth

🎯 Milestone: Complete set of 20 primary teeth by age 2.5–3

  • Brush teeth twice daily — parents should do the brushing until age 3
  • Use a rice-grain-sized amount of fluoride toothpaste
  • Wean off the bottle by 12-14 months to prevent bottle decay
  • Limit sugary snacks and sticky foods between meals
  • All 20 baby teeth should be in place by age 3
  • Watch for thumb-sucking — it's normal but should stop by age 3-4

3–6 years

Preschool & Kindergarten

🎯 Milestone: First permanent molars start forming under the gums

  • Increase toothpaste to a pea-sized amount
  • Teach your child to spit out toothpaste (not swallow)
  • Children can start brushing on their own but SUPERVISE until age 6-7
  • First dental X-rays may be taken to check for hidden decay
  • Dental sealants can be applied to molars for cavity protection
  • Establish a brushing routine: morning + before bed, 2 minutes each
  • First orthodontic screening can be done around age 5-6
📚
6–12 years

School Age — Mixed Dentition

🎯 Milestone: A mix of baby and adult teeth — the 'ugly duckling' stage is normal!

  • Baby teeth start falling out — lower front teeth first (around age 6)
  • First permanent molars appear behind baby teeth (don't fall out!)
  • Continue supervising brushing until they can tie shoelaces independently
  • Use a mouth guard for sports — dental injuries are very common at this age
  • Discuss orthodontic evaluation around age 7-8
  • Limit sports drinks, cola, and juice — they erode enamel
  • Sealants on permanent molars are highly recommended
🎓
12–18 years

Teens — Permanent Teeth

🎯 Milestone: Wisdom teeth may start appearing between 17-21 years

  • All 28 permanent teeth should be in place by age 12-13
  • Braces or aligners may be recommended — the best time for orthodontic treatment
  • Wisdom teeth monitoring starts around age 15-16
  • Electric toothbrushes can improve brushing quality
  • Avoid oral piercings — they chip teeth and cause infections
  • Address teeth grinding (bruxism) — often linked to stress and exams
  • Whitening treatments should wait until all adult teeth are in
  • Custom sport mouth guards for all contact sports
